DJ Service Contract
This contract is for your benefit as well as ours. Two copies of the contract will be signed. There is a $50 deposit on
this contract which is non-refundable to secure my service for the date of your event. The $50 deposit will be deducted from
your bill at the event. If you are to cancel, please notify me within 3 days before the event. If you cancel after this
period you will be charged the full amount for services rendered. If you live outside the 10-mile radius of Adams Ma you
will be charged for gas mileage. I will arrive 1 hour to a half an hour before the event. I may bring 1 assistant, but this
is not always the case. If any damages/theft occur to my equipment or myself or people in my employ at the event, you will
not be held responsible, only the parties involved with said damage/theft. This includes children in which case their parents
would be held accountable. If for any reason I cannot attend the event a suitable replacement will be sent in my place.
This person may be in my employ or from an another Disc Jockey service. If this occurs you will be notified prior to the
event. Please remember to keep your copy of this contract.
